Thank you for requesting our cat nail trim service. This service is available to adopters of cats from the Irvine Animal Care Center. We have updated the requirements for this service, and want to make you aware of these changes.

Vaccination and Health Requirements

• Proof that your cat is up-to-date on the following vaccinations:
  • Rabies
  • FVRCP
• Your cat cannot display symptoms such as:
  • Vomiting
  • Diarrhea
  • Nasal or ocular discharge
  • Skin disease such as ringworm or mange
  • Be under treatment for any of the above conditions

Fees

• $10 for cats and kittens adopter after July 1, 2017
• Free for cats and kittens adopted before July 1, 2017

By signing this document, you understand:

1. The Irvine Animal Care Center is a municipal animal shelter. The Irvine Animal Care Center is responsible for the care and housing of stray and owner-relinquished dogs, cats, and rabbits.
2. These animals have come to the Irvine Animal Care Center with a variety of health conditions.
3. Although the Irvine Animal Care Center uses best practices for hygiene and cleanliness, there is a risk that by bringing your cat into the center for the cat nail trim service that they may be exposed to diseases contagious to cats.
4. Our staff has been trained to handle your cat in the most humane, least stressful manner. However, trimming your cat's nails will involve some level of restraint of your cat by our staff. This restraint can pose a risk of injury to your cat. 
5. In the unlikely event that your cat bites a member of the Irvine Animal Care Center staff, the cat must be placed on a California state-mandated 10-day quarantine.
6. I release the Irvine Animal Care Center, its employees, and its volunteers from any claim, cause of action, or liability directly arising out of, or in connection with, my cat's participation in the cat nail trim service.

Such information is being requested and will be utilized by the City for the specific and limited purpose of future City correspondence regarding the subject-matter of this form. Pursuant to Measure S, an initiative ordinance passed by City voters in 2008, all information provided on this form will be kept confidential. Unless you expressly indicate to us otherwise or unless compelled by a court order, it will not be shared with other agencies, businesses or individuals.
