Open, complete and print the Program Registration Application

Sign the Liability Waiver (application will not be processed unless signed).

Fax the application with payment information (MasterCard or Visa) to 949-724-6608.
Please verify information is legible and the application includes a complete credit card number and expiration date.
Your registration will be processed in order received. If you have not received your receipt within one week, or you have questions, comments, or suggestions, please contact Program Registration at 949-724-6610 or by e-mail at QuickReg@ci.irvine.ca.us.
